The new approach to Special Educational Needs (SEN) will focus on ensuring co-ordinated services are planned across different services for education, health and care. This section of the Local Offer is being developed to provide information on Health services for families and young people with SEN.
Health services provided within the Stockton-on-Tees area are commissioned (purchased) by a number of different organisations, including:
• NHS England
• Clinical Commissioning Groups (CCGs)
• Council Public Health Departments.
Each commissioned service will have its own:
• agreed eligibility criteria (who can access the service)
• agreed referral criteria (how to access the service)
For information about those services offered through the Hartlepool and Stockton on Tees Clinical Commissioning Group Strategies and Plans click here.
If you have any questions regarding any of the health services mapped in the Local Offer, and whether your child might benefit from referral, please discuss these with your GP or any of the existing health professionals involved with the care of your child.
If you require further information regarding personal health budgets the personal health budget document listed is available.
