Care and Treatment Reviews (CTR)
Care Treatment Reviews (CTR) were introduced in October 2015 as part of Transforming Care; NHS England's commitment to improving care of people of all ages with learning disabilities and/or autism. <SEND>
The aim is to reduce the need for admission, prevent unnecessary lengthy stays in hospitals and reduce health inequalities. This enables the individual to live in community settings that provide the greatest opportunity to realise their potential and independence.
In Stockton-on-Tees these reviews are supported by a multi-disciplinary team of representatives from NHS North East North Cumbria Integrated Care Board (ICB), Tees, Esk & Wear Valleys NHS Foundation Trust and Stockton Borough Council.
