Children's Care Co-ordinator (life limiting conditions)
The Children's Care Co-ordinator will support you from the point of diagnosis. They will help you get the support and advice at the right time from the health system, the social care system and the education system. <SEND>
The co-ordinator will ensure that your child's needs are met and your voices are heard. The support will be personal and as flexible as possible to provide you with a point of contact to support you through this journey. They do this by working with:
- your family
- providers
- the local authority
- the voluntary sector
They will use the personalised care approaches and support the transition to adulthood.
