When people come out of hospital, they can need more support than usual to help get them back on their feet. Five Lamps' Home from Hospital project provides this support for up to 14 days post discharge.
People often lose their confidence after being in hospital. Our staff can provide a range of support:
- Shopping
- Emotional Support
- Prescription Collection
- Back into the Community
- Social Visits
- Appointment transport
- Liaise with a wide range of other services
