We provide a range of 0-19 health services within Stockton-on-Tees. <SEND>

0-19 Service

Health Visiting

Our Health Visitors deliver the Healthy Child Programme. We offer mandated contacts to assess, monitor and promote the development of children aged 0-5 years old. Support can be provided on a wide range of topics including infant feeding, maternal mental health, growth and development, sleep, speech and language and behavioural concerns. Onward referrals to specialist services can also be undertaken to meet the needs of your child/family.

5-19 Service

Our 5-19 Team provide support for young people up to the age of 19 years (25 years for an identified SEND need). Our 5-19 Team can offer a holistic health assessment to ensure appropriate support can be put into place. We assess and monitor development and can also provide support around, head lice, toileting, healthy eating, sleep, continence and any other general health need. We offer 1:1 intervention as well as group work.

Special Educational Needs and Disability (SEND) Team

Here in Stockton we have a dedicated SEND Team which focuses on ensuring children's needs are identified at the earliest opportunity to enable specialised support to be put into place. We work closely with partner agencies, supporting the Stockton SEND Strategy. We also support children and young people who require a Education Health and Care Plan. The SEND team complete targeted work within a case.

Early Intervention and Resilience

The team offer support to young people around resilience, sexual health and reducing the impact of low level drug, alcohol and vaping. This can be undertaken at school or in the community and can be either 1-1 or group work. We also attend community events throughout the year.

Emotional Resilience

Our Emotional Resilience Team can provide low level brief intervention support around the management of emotions, anger, exam stress, relationship breakdown, self esteem and confidence. Support is provided 1:1 alongside the completion of a family health needs assessment to identify any further need.

Growing Well Growing Healthy (GWGH)

This is an exclusive service which supports children, young people and their families in preventing and treating childhood obesity through positive behaviour change. The GWGH Service offers bespoke intervention to support, identify and address lifestyle habits for children aged 5-19 years. The GWGH Service also includes the delivery of the National Child Measurement Programme (NCMP).

Infant Feeding Team

Our Infant Feeding Team focus on 0-4 years and offer support to new parents on the benefits of nutrition from your child's earliest days. Our Infant Feeding Team are dedicated to offering a wide range of services from infant feeding support to weaning, nutrition and activity advice as children grow into their school years.
