When applying for a National Disability Card, you will be asked for evidence that shows you have a disability, for example:
- the back of your Blue Badge card
- a disabled persons' Freedom Pass
- a disabled persons' bus pass
- a letter from your doctor or other relevant medical professional
- Disability Living Allowance (DLA) decision letter
- a War Pensioners' Mobility supplement letter
- a PIP decision letter
- any other document that shows you are disabled
